That's a good way to get someone hurt. When I hear go hard and heavy I'm thinking 3-4 reps with failure as my measuring stick.
There was nothing inherently wrong with the advice PGD gave. Hard and heavy is a subjective description, at best, and if someone took that to mean load up the bar and do 3 reps, then they are a ding-dong and probably would figure out how to get themselves injured on their own without the internet advice
