    Bentley UltraRunning Project Survey (BURPS)

<label><label>Thank you for taking the time to respond to the Bentley UltraRunning Project Survey (BURPS). This survey is meant to provide information on the culture of the ultrarunning/trail running community and those who participate in it. The data from the academic project will be used to help describe ultrarunning and trail running culture.</label></label>

The survey should only take 10-15 minutes to complete. You will be asked general demographic questions, activity questions, and attitudinal questions about ultrarunning and trail running. All participation is of course voluntary, and all responses are anonymous and confidential. No personal identifiers will be collected, thus all your responses are confidential and your participation anonymous. All data will be analyzed in the aggregate. If at any point you would like to stop your participation, or skip a question, you are free to do so.

If you have any questions regarding the survey, and how the data will be used, you can contact Gary David, Department of Sociology, 175 Forest Street, Bentley University 02452. You can email at: gdavid@bentley.edu. If you have questions or concerns about your rights in this research, you can contact Mary Louise Pauli (IRB Administrator), Bentley University Institutional Review Board, 781-891-2660. Email mpauli@bentley.edu.

Thank you so much for your time and support of the survey, and we look forward to seeing you on the trails!

    Uh oh... now what?

On a personal note, they don't list "UltraRunning" -- one of my favorite magazines.

Apologies all around.  I contacted the e-mail listed, correction was made.

They seem to have overlooked the 49th and 50th states the first time through, but they're there now - just down at the bottom, out of alphabetical order.
