I'm back...with good news and bad news...
Good news: nothing wrong with my knee
Bad news: nothing wrong with my knee
I'm not really all that surprised, actually. The PA manipulated my knee, sort of twisted my leg both directions with knee bent, applied pressure, bent and straightened my knee, and really couldn't find anything structurally amiss or any signs of knee weakness or instability. He's in agreement that what I'm dealing with is almost certainly an overuse thing in the form of "runner's knee" or perhaps some sort of bursitis or tendonitis (which he admitted can take months to heal, especially if one is still continuing with the activity that caused the issue). I asked if ITBS could be an issue, but he didn't think my symptoms fell quite in line with that.
The fact that I am able to do shorter runs without any problem and am pain free at least half of the time and really only have pain after 6 miles or so on longer runs had him believing that I'm irritating and re-irritating something, not that I have any meniscus issues...an injury like that would have me in pretty much constant pain and would prohibit running entirely. He doesn't think running with the pain is doing any sort of damage, other than making me really hate my knee and dislike running for a few hours each week.
He's recommending that I scale back on the miles I had been doing (which I'm already doing), continue with the quad work, bike if it's not causing pain, and consider having my gait/shoes reassessed, though he thinks it's not likely a shoe issue, since I've been in essentially the same shoe for 9 months and only had issues for the past 2.5.
He also said that he would refer me to a PT if I wanted, but that there's not likely much a PT would have me doing that I'm not already doing. He'd also schedule an MRI, but that he really doesn't believe it's going to show anything amiss, which has been my suspicion.
So...I guess it's mostly a good prognosis. I'll continue to train to the best of my ability and if it gets to the point where things are not getting any better even with more conservative training and cross-training, then I will probably suck it up and take as much as a month off...and forget the marathon for now. He didn't recommend that I stop running entirely or forget the marathon goal, so that's promising.

Congratulations.....I think. Sometimes its better to know exactly what's wrong and deal with it than to try to keep fighting through the pain. The good news is that the PT didn't tell you to stop, so you still have a shot at continuing your goal, even with a less aggressive plan. You're up to about 16 miles already, so you don't have that far to go and you have a solid base so I sure wouldn't give up just yet.
I'm in the cutting back on training mode as well and am still having some problems with my achilles tendon. I'm planning on running a 10 mile race on Sunday, so that should give me a good indication on whether I can keep up my training plan.
Keep us up to date on how you're doing and hopefully I can buy you a beer after the race in October when we both finish.

Heh, even if I don't run Milwaukee, I will be there. My best friend (backroadrunner) is also registered, so if I'm not physically running I will have a big HTFU sign!
Achilles stuff is miserable. I have had random problems with that a couple of times, but never anything too severe. Even the mild issues are awful, though. I hope your injury works itself out fast!

i'm glad you went to get the durn knee checked out even though you sorta kinda had an idea that the knee was just telling you it needed some time off. those knees. we really put them to work.. when they need time off, they usually let us know
it sounds like you have a good plan here and will take as much time as you need which is very smart. theres nothing like turning Nothing is Wrong into a Something is Seriously Wrong injury by continuing to train through moderate to severe pain/discomfort.
i wish you nothing but a speedy recovery. if not milwaukee, there are certainly plenty of other marathons for you to run
p.s. can you clue me in as to what HTFU means? thanks.

Yep, I'm going to run the Cudahy race. The last time I ran it was in 1992 (I guess I am old) and I'm tempted to wear my old t-shirt from back then. I'm planning on taking things easy during the race and will be using it to get a good gauge about my conditioning.
I actually just signed up last night, before I saw that it's supposed to be in the 90's this weekend!

i know. its now showing 89F.
like you, the race will really be just a training run for me because 1) i'm coming back from an injury and also 2) i have a big "A" race the weekend after, CDC 1/2 mary so i don't want to kill my legs this weekend.
i also plan to tag on 3 miles before and after for a total of 16.

Someone from my running group is doing the cudahy race. I am doing the Brat Days, 4 mile Brat Trot in Sheboygan though.

how is your knee been holding up these past few days? what kind of activity have you been doing? i'm hoping to hear some positive news!

Ha, I could never be upset over a thread hijacking...I'm the queen of the art!
Knee is feeling much better. I'm sure scaling back on my training has helped. It's still a little "off," though. I have been on a quest to find the perfect shoe. 2 days ago went shoe shopping and it was between the Nike Structure Triax 11 and the Asics GT-2130. Didn't "love" either shoe and basically went home with the Asics with the thought "hey, it's the top-selling shoe in the world, it has to be good, right?
Saleswoman talked me into the medium width, since my heels are so narrow and the forefoot on both models are pretty roomy. But my heels still felt a bit of slippage.
Ran 4 miles in the shoes yesterday...could tell by 2.5 miles in that they weren't going to work. Too much heel slippage and my pinky toes were hurting (probably because the shoes really weren't as wide as what I think I need), as well as my outer arches. They were also a little stiff through the forefoot, so I knew where the arch pain was coming from. This is one reason I ended up in the NB 902/903. First shoe that really didn't irritate my arch and make me feel like I'm fighting the shoe.
So made the hour long trek back to the store today to return them and try a few other models. Ran into that dumb heel thing in just about every shoe I tried. I wanted to try the Mizuno Inspire 4, but they were out of the 8 medium and wide. Tried the Nirvana (I think that was the model) in a medium and really liked the heel fit and arch, but I definitely needed a wide, which they didn't have in stock. Also didn't want to pay that much for a shoe if I can avoid it (It's a lot more shoe than I really need or want).
Got home and called around to a couple other running stores...no one carries the Inspire in wide. Sheesh. Sounds like a lot of companies are ready to roll out new models, so stores seem to not have the selection of stock.
Based on the fit of the Nirvana and how light and flexible the shelf sample Inspire felt when I handled it and flexed it I decided to take a chance and order the Inspires from Running Warehouse (got 2 sizes, because Mizunos can often run small, so I wasn't sure if I need the 8 or 8.5)--their price was over $20 cheaper than the local stores are charging, anyhow. Paid extra for faster shipping, so hopefully I will have them by Monday.
*sigh* What I'd give to have "normal" feet....

i like the inspire. I alternate between it and the wave rider. it would take an act of god to get me out of Mizuno. Good luck.

Even though I wasn't able to try on the Inspire, I loved the fit of the Nirvana, though it was too narrow through the forefoot. If they made that in a wide I maybe would have considered it. I believe the Nirvana and Inspire share that same heel technology...it's like some sort of elastic device that keeps the heel from slipping--BINGO. I think that is exactly what my heels need. I apparently have a really weird, small heel, as I have a bitch of a time finding shoes that don't slip. NB is, thus far, the only manufacturer that works for me and I have even had a couple of NB shoes that gave me issues. The Saucony heels are also good for avoiding slippage, but come up a little too high and curve in too aggressively and gets my achilles.
Funny...the guy helping me at the store had a RAW spot on one heel. He had tried the Nike Structure Triax 11 in a narrow and it was loose on his heel. Glad I didn't go with my second choice from what I tried on Weds. I really think I would have been unhappy with that.
Hey, how do you find the Mizuno to wear, mileage-wise? Part of what has kept me from trying them in the past was the reputation for wearing out fast, but it's not like my NB 903s are a high mileage shoe (I retired my first pair at around 335 miles and should have ditched them by 300, in hindsight), either, so I don't think I'm going to lose anything.
