Knee soreness with no injury (Read 123 times)

    Hi All,

    I ran a half marathon in 2016 and after the race and ever since I’ve had a nagging knee soreness.  Last fall I did 8 weeks of physical therapy and got a knee MRI which indicated patellar maltracking but nothing torn ripped or otherwise damaged.  The doctor and physical therapist advised that I can run with a brace while strengthening my hips and core.  I’m writing here because I’ve been trying to run with minimalist shoes because I’ve read they can help with knee pain but it hasn’t made a difference with short 1 mile runs.  I’m wondering if I should choose a more supportive shoe while strengthening everything else. 

    Does anyone have any recommendations for shoes or other suggestions for helping with patellar maltracking?  Thanks!

      Did PT help? Have you had your running form analyzed? You have to correct the cause or it won't go away. You should not have to depend on a brace to run. This is not the right strategy. Ditch it. If too painful to run without it then you shouldn't run. You may need another opinion or another PT combined with you simply need time to heal and resolve while not running. 4 years of soreness means you haven't solved anything.

        Thanks so much!  I don't think PT helped too much and I haven't had my form analyzed.  How can I have my form analyzed because I'd love to do that?  I'll try a run without the brace this week in more supportive shoes.  Do you think I should see another orthopedist?

          In my experience, strength work is very important for knees. add bodyweight squats and see if it makes a difference (obviously if your knee allows.)

            If dealing with a condition for 4 + years I would be looking to see another ortho and PT. Many sports PT centers will do a running form analysis. For sure the ones in bigger hospital systems. You need to make calls. I know a great source online that will do this virtually if you prefer and analyze you. Reach out to me if you want her contact info. She is great. She has her own Facebook injury group, does weekly podcasts and she is a runner which I think is very important. Her focus is to always figure out the exact cause of the issue. You can't solve your issues until you determine this. This is how you treat.

              My experience as well. I used to run but developed PFPS. I stopped and started jogging for years and could not get rid of PFPS (runners knee). Last fall I picked up new shoes to provide some fresh cushion and I added lunges, leg curls and general core and leg work to my PT routine. This has made life better for me. Going down stairs is better.

                I agree with Tchuk 100% as I had many mechanical issues in my early years running, IT band for a whole year, shins for a whole summer season and knee problems after a fall caused by a dog. After rehab, a sport injury doc recommended me to some specialists in bio-mechanics of running at a university in my city. They worked with me for a 6 week stretch and changed my bio-mechanics (running form)considerably with impressive results. I have been running for the last 10 years with very few issues and I run 30-40 miles a week. I will admit that I work my core on a daily basis as maintenance but it's the price to pay at my age... Good mechanics beat braces hands down, the effort at correcting your bio-mechanics is a worthwhile endeavor.

                  A 4 year problem? I would go back to a sports doc or physio. Dr Google is not going to get you out of this or you would have already fixed it.

                        you have to get another opinion, i had some knee pain but went to an Ortho who happened to have contact with a great PT who works exclusively with runners, she was amazing. she correctly diagnosed a very minor ankle issue that was putting some pressure on my knee when i landed in my stride, i worked with her for a month and she fixed everything up, it makes a huge difference on the PT that you see. definitely seek out the PT's whose main experience is with runners
