It comes up with fatal error when I try add new equipment

When I click on new equipment it comes up with

I understand that is the error message, but that wasn't what I am looking for. I need to know what you have typed into the form. I need to enter the exact values that you have entered in order to reproduce the problem. You can take a screen shot of the page and send it to me, which might be easier. I have sent you a private message with my email address.
