It's not a race nor test to prepare for. They will increase incline and speed to your level of fitness to get your heart rate up to a certain rate, no more, no less.

It's not as fun as it sounds. It's an expensive race, no shirt, not even a bagel when you're done. I had mine on April 1st, and lasted only 14 minutes, but was also sick that day and didn't feel like proving anything to anybody. If you really want to train, I'd get on the TM, and move the incline up one mile, speed the next mile, incline the next miles etc.. But seriously, take it from someone who's been bare chested in the ER with the crash cart beside me, listen to your doctor.
