    Hey, Marcus - what's the latest on your knee? I decided (again, lol) to stay out of the swamp, so I haven't kept up with you.

          When I was in college my girlfriend called me and asked me to go to a webpage and confirm whether it was a picture of me or not. Sure enough, the picture they'd found was of me. I'd had no idea that picture had been taken, had been put online, and have no idea if it is still there or not. It was a picture of me wearing some shorts w/a gigantic tear in the rear end w/nothing underneath when I'd forgotten they were torn and needed to throw something on quick after getting out of the shower. Good cheek shot. I wonder if I could find that picture somehow...
          OK, all beutiful women we must cheer Marcus up.

          The Greatest of All Time

            Preliminary MRI results show bone marrow edema. I will have the final radiologist's report today. As part of my job, I regularly review MRI results from backs, knees, necks, and shoulders and I had never read a finding of bone marrow edema before. I had no idea what the hell it was. Essentially it's a stress reaction that is often times asymptomatic. I guess it's in the same injury family as a stress fracture but probably not as bad. Treatment appears to be limited weight bearing, i.e. no running for about 4-6 weeks. This has not been confirmed by a doctor yet. I set up an appointment for Monday morning with an orthopedic surgeon. I am going to take him my MRI films (they are currently sitting on the credenza in my office) and see how he reads the MRI. From my experience, specialists sometimes read MRI's differently than radiologists. I see this all the time with neurosurgeons. So, I will know more on Monday morning. This was my third week of very limited running and I think it's getting better. I am pretty sure I have this condition in both knees although only a right knee MRI was preformed so it's speculative. The symptoms are pretty identical though. In the meantime, I will just continue cross training and try to focus on pulling out a good fall marathon as I am already signed up for Chicago and still think objectively I could go ~2:50 if I can resume running in about 4 weeks from now. I can maintain aerobic fitness and actually improve leg strength and flexibility so it's not like this injury required a dead stop. At least as of today. Thanks for asking Teresa.

                    Huh...so your bone marrow is bloated, essentially. I have never heard of that, either, but I can see how that could be painful. I had horrible edema when I was preggers...so bad that I have stretch marks on one of my feet. My skin hurt, but skin is flexible. I can only imagine what swelling inside the bone (I'm guessing this is what you are feeling) would be like...yikes! Shocked

                          I know a lot of you don't go into the Swamp, so here's the latest from Marcus.
                          Had my appointment with an orthopedic surgeon this morning. Nice guy, athlete (runner & cyclist), and works on a ton of collegiate athletes in the area and some professional type folks too. He took a number of x-rays and reviewed my MRI films. Diagnoses: 1. Patello femoral pain syndrome (runner's knee) 2. Bone marrow edema of the proximal tibia (lower knee) per MRI. The bone marrow edema is a precursor to a stress fracture as I thought. He gave me some Celebrex and Ultracet (tramadol) for pain. Since this is starting my 4th week of little to no running, he said I should be able to run again gradually in 1-2 more weeks, so that's cool. The bone marrow edema, like a stress fracture, will resolve itself. Now the bad news: the runner's knee is never going to completely go away. As long as I run, it's going to be there to some extent. My doctor trained under one of the high gurus of runner's knee so I trust he knows what he's talking about. He said I will learn how to manage it and adjust volume and intensity as appropriate. In other words, very soon I will learn what I can handle and what I cannot. He said it may get to a point where it's too much of a hassle and I will just stop running and start seriously cycling again. Maybe. I really love cycling almost as much as running, but don't live in an area where I can ride like I used to down in Austin. He said athletes like myself are the worst patients to get this crap because we don't like to take it easy and usually just make it worse and then wonder why all of a sudden we cannot run. But there is no definitive answer to how much volume or intensity I will be able to handle. It's basically how much pain do I want to put up with, or at what point do I do more harm than good. But, this runner's knee will not cause arthritis nor will it require surgery. I am just going to have it forever. So it's another week or so of cross-training only and then hopefully a very gradual return. Objectively it is feeling better and yesterday it almost felt good enough to run a little bit but I didn't. This sucks.

                                Thanks for the vote of confidence. Unfortunately at this point it's not about proving the docs wrong, it's about listening to my knee. If it says "dude, no" then I won't run. Since this all flared up, I have noticed when I have ran on a painful knee my mechanics go to crap because I change my stride to attempt to compensate for the painful knee(s). This is a really bad idea and I won't do it any longer. I think it's getting better every day. Last night and this morning as I was leaving the gym I ran the 100 yards to my car just to see how it felt and it's not quite there yet. Until then, cross-train, stretch, strengthen, and ice.
